I sent a rant off to a hospital billing company. Long story short, my old job was about to end, my employer no longer had workmans comp paperwork, and my company health insurance expired shortly before I stepped on a nail removing the interior of the old facility. Worried about tetanus, I went to the emergency room and it was expensive.
"... Finally, I'm not sure why a 0.5 cc dose of tetanus vaccine costs $489.00 to inject, but I'm confident that is the most expensive product I've ever had so little of. However, if we consider that the hospital personnel need to get paid for their 3 minutes of attention, I suppose it is reasonable as long as they are being paid $9,780 per hour."
Anyway, I ranted to them so I thought I'd share with you.

In Indiana we are required to provide W.C. regardless of anything. I really feel you could win this by making your old employer pay for the bill, even if he has to pay for it personally. That's Indiana law though. I'm not sure how that affects California.
This is from personal experience. We had the state hammer us one time for not providing (long story). Our ignorance was very costly.

Freakin' eBay & PayPal with their hold payment funds for up to 21 days to clear. Maybe sooner if positive feedback is received or 3 days after delivery is confirmed.
I mean its one thing to protect buyers; its another to set up a system where they'll gain interest on your money while it sits!
Ugh! Caca poopoo!
